- Simply email help@journeyhealthcare.com with your name, your current DPC and your preferred new DPC (if you have one in mind)
- Transition Process: Once notified, Journey Health will assist in transferring your care to your new DPC. This may involve coordinating with your new provider to ensure a smooth transition of your medical records and care plan.
- First Appointment with Your New DPC: After switching, schedule an initial appointment with your new provider. This visit is crucial for establishing your medical history, discussing ongoing treatments, and setting health goals.
